As a general rule, California doesn't require witnesses (e.g. on Deeds, etc.), so the only witnesses I keep track of in my journal are those used for identification, i.e. Credible Witnesses, which is required in CA.

Yes, your name must be in there as well as the title- notary public, however it has much more wrong with it than the simple item you mentioned.
