Tan Tock Seng Hospital's Integrated Care Hub (TTSH-ICH) is the rehabilitative pillar of HealthCity Novena, focusing on bridging patients from hospital to home. It offers patients a seamless continuum of post-acute care to prepare them for reintegration back into the community.

The last two buildings, the revamped and expanded National Skin Centre and the Tan Tock Seng Hospital Integrated Care Hub are set to open in 2023. The second phase will map out new care models for outpatient and ambulatory care, in line with TTSH's expanded mission of population health. More details will be released soon.
