Dinamakan sempena pahlawan legenda Melayu di zaman Kesultanan Melayu Melaka abad ke-15 iaitu Laksamana Hang Jebat. Kapal perang jenis frigat moden ini dibina oleh Yarrow Shipbuilders Ltd, Glasgow, Scotland dan dilancarkan pada 27 Mei 1995 oleh Datin Seri Dr Siti Hasmah binti Mohd Ali, isteri kepada Perdana Menteri Malaysia.
