Cyrius is a tool to genotype CYP2D6 from a whole-genome sequencing (WGS) BAM file. Cyrius uses a novel method to solve the problems caused by the high sequence similarity with the pseudogene paralog CYP2D7 and thus is able to detect all star alleles, particularly those that contain structural variants, accurately.

Cyrius, a CYP2D6 ‐specific tool, demonstrated the most robust performance, achieving the highest concordance rates for CYP2D6 in all instances, comparable to the consensus approach in most cases.

Here we describe a novel bioinformatics method, Cyrius, that accurately genotypes CYP2D6 using whole-genome sequencing (WGS) data. We show that Cyrius has superior performance (96.5% concordance with truth genotypes) compared to existing methods (84-86.8%).

CYP2D6 metabolizes 21% of clinically used drugs. Cyrius resolves common SVs and sequence homology to paralog and genotypes CYP2D6 with 99.3% accuracy.
