A volar intercalated segment instability (VISI) deformity features a volar rotation of the lunate mainly after disruption of the lunotriquetral ligament. On lateral imaging, volar flexion of the lunate with a scapholunate angle , measuring <30 degrees and an increased capitolunate angle measuring >30 degrees 1 has been used in the diagnosis.

With attenuation or injury to the dorsal intercarpal ligament, volar intercalated-segment instability (VISI) pattern follows; this can be visualised on lateral radiography. An LTIL tear most commonly results in a VISI deformity. Not only a loading type of trauma, but a rotational force to the wrist can also result in ligamentous injuries.
How can an understanding of wrist motion lead to better treatment and rehab? Why is it important to understand carpal motion? Can the proximal row be dynamically stabilized? Pain with resistive activities, able to work with moderate ache, and able play golf. power forward, center position etc.. Or we will never know.... Why this works?

On a lateral radiograph of a normal wrist, the capitolunate angle measures less than 30°, and if the angle measured greater than 30°, it would represent LTL rupture and VISI . VISI is considered a CID lesion when there is a traumatic etiology and CIND lesion if rheumatic.
