NYC DOB Updates Refund Form Requirements for RF1 Submissions
Nov 22, 2023 · The New York City Department of Buildings (NYC DOB) has issued a significant update regarding the RF1 form, which is used for refund applications. Beginning on April 1, 2024, only RF1 forms marked with the revised date “3/24” in the lower right corner will be accepted for refund consideration.

For initial registration or updating of member data record and/or declaration of dependents, require the employee/s to properly accomplish the PhilHealth Member Registration Form (PMRF).
The RF1 form is submitted to initiate a review of supporting documentation for consideration of a refund for money previously received by the Department of Buildings. NOTE: Handwritten forms will not be accepted.
