High-grade prostatic intraepithelial neoplasia (PIN) is now accepted as the most likely preinvasive stage of adenocarcinoma, almost two decades after its first formal description. High-grade prostatic intraepithelial neoplasia (PIN) is considered the precursor of most cases of prostatic cancer. 1–3 This microscopic finding arises within preexisting ducts and acini and consists of a proliferation of epithelial cells with cytologic changes, including nuclear and nucleolar enlargement, that mimic cancer (Fig. 1).
