DD Form 1896, also known as the DoD Fuel Identaplate, is a form used by the US military to identify and track military fuel containers and tanks. The main purpose of this form is to ensure that fuel containers and tanks are properly labeled and tracked to prevent contamination, misidentification, or theft.

Use this form in the event a Foreign Government or Commercial Customer requests fuel and does not have a valid purchase device (DD Form 1896, AIR Card, etc.) at the time of the fuel servicing request. Except as noted, all of Part I, Part II or Part III, and Part IV are mandatory data entry fields at the time of the sale.

The pilot use the identaplates DD Form 1896 or 1897 for buying aviation fuel and lubricants from government sources. When used, the issuing activity will imprint the billing information on DD Form 1898.
