Users can print an AF 988 when they receive an email notification that the CSS or Base FM has authorized leave and assigned a leave number.

AF Form 988 is a legal document in the Air Force that requests and authorizes a leave from work. You need to complete all three sections when requesting a leave.

Follow these steps to submit your leave paperwork. Download the AF Form 988. You may need to open in Acrobat for the form render properly. The most recent Form 988 was published 20 Nov. so try to use the most up-to-date (previous edition can also be used). Fill out Section 1 of the form according to the example.
