Incision and drainage is an effective method for treating fluctuant buboes and may be preferable to traditional needle aspiration considering the frequency of required re-aspirations in the study patients.

Apr 17, 2023 · Inguinal bubo is a sexually transmitted infection mostly associated with lymphogranuloma venereum (LGV) and chancroid. Diagnosis is made based on clinical examination. Antibiotics are given as the first line of treatment followed by regular follow-up.

In 90% of cases, the bubo is unilateral and can reach up to 10 cm in diameter; suppuration, sometimes with drainage, occurs in approximately 20%. Although the onset of fever usually coincides with lymphadenitis, it can precede the bubo by several days.

The goal of treatment is to cure infection and prevent ongoing tissue damage, although tissue reaction to the infection can result in scarring. Buboes might require aspiration through intact skin or incision and drainage to prevent formation of inguinal or femoral ulcerations.

After informed consent was obtained, patients had buboes drained by needle aspiration or by incision and drainage with packing according to a random numbers table. Results: Twenty-seven patients were included in the study. (22 men and 5 women), with 12 randomized to incision and drainage and 15 to aspiration. The mean age was 35.7 ± 13 years.

Some cases may require longer treatment than the 14 days recommended above. Fluctuant lymph nodes should be aspirated through healthy skin. Incision and drainage or excision of nodes will delay healing and is contraindicated.

Aspiration of fluctuant buboes using a wide bore needle through adjacent healthy skin every second or third day. An incision and drainage should not be attempted. If buboes persist, the patient should be referred. Sequelae such as strictures and/or fistula may require surgery.
