Patients with primary immunodeficiency disease (PID) are not only vulnerable to mycobacterial disease, but are also more likely to develop adverse events following BCG vaccination. These events can range from regional disease (BCGitis) to disseminated disease (BCGosis).

Recent advances in molecular biology help to distinguish BCG disease from other mycobacterial infections, especially from tuberculosis.

In this study, the clinical and immunogenetical features in a cohort of Chinese patients with BCGosis/BCGitis were investigated. For the patients with abnormal immunological functions, Sanger sequencing was used to identify the involved genes. There were 74 confirmed cases of BCGosis/BCGitis during 2007–2012.

CONCLUSIONS: Cutaneous BCGitis is a rare reaction secondary to vaccination, which manifests locally as granulomatous polymorphic lesions, whose therapy includes topical treatment and/or oral antibiotic therapy. KEYWORDS: BCG; Bacillus Calmette-Guérin; Tuberculosis; Vaccination; Antibiotic therapy.

Bacillus Calmette–Guerin (BCG) is a live attenuated vaccine with the potential of causing severe iatrogenic complications in patients with primary immunodeficiency diseases (PID) before and after hematopoietic stem cell transplantation (HSCT). We aim to investigate risk factors of post-HSCT BCG-related complications in PID patients.

Se entiende por “BCGitis” (BCG-IRIS en caso de personas inmunodeprimidas) a la linfadenitis por vacunación con bacilo de Calmette-Guérin (BCG) es la complicación más frecuente de esta vacuna. Se define como el crecimiento de los ganglios linfáticos regionales ipsilaterales posteriores a la vacunación.

La vacuna Bacillus Calmette-Guerin (BCG) que se administra a los recién nacidos de países con alta incidencia de tuberculosis puede ocasionar reacciones locales hasta infección diseminada en pacientes inmunocomprometidos.

La BCGitis es una complicación rara de la vacuna de Cal-mette – Guerin, pero cuando aparece, es una real emer-gencia médica, que debe ser tratada de inmediato.4 Su frecuencia ha disminuido en los últimos años, pro-bablemente debido a la mejor técnica de aplicación, así como a la disminución de la dosis.
