The FDA has issued a recall on frozen supplemental shakes that were found to have been contaminated with Listeria monocytogenes.

The supplement shakes were sold to hospitals and long-term care facilities, including nursing homes. There is a nationwide recall for the ReadyCare and Sysco Imperial Frozen Supplemental Shakes. Twelve deaths associated with the listeria outbreak have been linked to the shakes.

The FDA says an onsite investigation at Prairie Farms Dairy revealed three environmental swabs taken in the processing area tested positive for listeria monocytogenes.
